What is COPD?


Chronic obstructive pulmonary disease (COPD) - is a disease that makes it difficult to move air into and out of your lungs. The airways of the lungs become inflamed and become obstructed or blocked. Taking a breath becomes difficult.

         COPD is and umbrella term for two respiratory diseases:

1.Chronic Bronchitis

2.Emphysema

Some people with COPD suffer from both chronic bronchitis and emphysema. Both can make it difficult to breathe and can cause breathlessness.

What are the differences between the two?
Chronic bronchitis is an inflammation of the airways in the lungs and leaves them irritated. Because of the irritation, the glands in the lungs produce large amounts of mucus that block the airways. This causes you to cough, spit and makes it hard to breathe.

Emphysema damages the tissue in the lungs. The lungs become non-compliant or limp. Normal lung tissue can expand and snap back - like an elastic band. This stretch allows you to exhale air from your lungs after you’ve inhaled. If you have emphysema, air becomes trapped in your lungs, this makes it hard to catch your breath and you feel fatigued. 


Is COPD and asthma the same?
Although COPD and asthma have similar characteristics such as coughing and wheezing, they are two distinct disease conditions. They are different in disease onset, symptoms and how often they occur or frequency.

The onset of asthma typically occurs during childhood or adolescence. COPD most often develops in smokers who are in there mid 40s. Exacerbation's of asthma or asthma attacks are characterize by wheezing, shortness of breath and coughing, chest tightness. Asthma often has identifiable triggers such as allergens, cold air or exercise. However COPD attacks or exacerbations are usually caused by a respiratory tract infection. Asthma patients who receive treatment have near normal lung function and are symptom free between attacks. COPD patients usually have some symptoms every day but can be dramatically reduced by avoiding respiratory tract infections by keeping the air around them as clean as possible. Bronchiodialters use smoking cessation and pulmonary exercises can also help.

What Causes COPD?


Smoking, occupational exposure to dust, and outdoor and indoor air pollution cause COPD. COPD results in damage to lungs over many years.

  • COPD affects both men and women. The average age is 60 years old. However COPD can be diagnosed in-patients in there 30s and 40s.
  • Smoking is the main cause of COPD. Only 10-15% of heavy smokers will develop COPD. It is still unknown why.
  • COPD can result from occupational exposure to chemical fumes and organic dust such as grain, cotton or wood and mining dust.
  • COPD is caused by outdoor and indoor air pollution such as heavy concentrations of mold, dust mites, and allergens in a patient’s home.
  • Outdoor pollution such as emissions from cars, heavy traffic and factory emissions can also be a cause.
  • COPD is more likely to occur in children who develop respiratory symptoms due to viral or bacterial infections.
  • COPD may occur in preemies whose lungs have not completely developed.


Who gets COPD?


In the United States 70% of COPD is due to smoking, 30% is due to previous lung infections or recurring asthma attacks that damage the lung tissue or genetic abnormalities like alph-1 anttrypsin deficiency. Even smoking related COPD tends to run in families, suggesting a genetic link.

 



COPD Symptoms


The main COPD symptoms are shortness of breath, a feeling like you are unable to get enough air, frequent coughing or wheezing.


 



Do you have COPD?


1. Have you smoked for more than five years?
2. Have you had many episodes of asthma or bronchitis?
3. Do you cough often and bring up heavy sputum?
4. Does your cold last longer than when you where younger.
5. Do you usually get bronchitis every winter.
6. Do you feel short of breath during normal activity?
7. Do you feel like you have something stuck in your chest when you cough?
8. Do you feel like there has been a change in your breathing.

Progression of COPD

COPD is also called the silent disease because symptoms usually progress slowly. At first you may only notice shortness of breath only during exercise. A morning cough may develop; these symptoms at first might not seem important but can progress to the point where activities of daily living can become very difficult.

COPD cannot be cured, but can be controlled. The progression and severity of symptoms can be controlled through:

1. Appropriate activity
2. Medication
3. Prevention of infection
4. Smoking cessation.

  •  COPD is the fourth leading cause of death in the United States.  In 1998 approximately 108,000 Americans died of COPD. COPD'S prevalence and death rate are rising. 
  • In 2020, COPD is projected to become the third leading cause of death in the United States.  
  • COPD has a higher mortality than asthma. The highest increase in mortality has been in white women, as observed between 1960 and 1998 an estimated 30 million Americans have COPD. However, only 16 million adult Americans have been diagnosed with the disease.
